Information you choose to share.
When you send an inquiry, we receive your name, email address, state, selected topic, and any optional message. We use this information to respond and arrange an appropriate conversation. Do not send medical information, Social Security numbers, policy documents, account numbers, or detailed financial records through this form. An inquiry is not an insurance application and does not create coverage.
Inquiry delivery and spam checks.
Formspree processes form submissions and delivers them to our business inbox hosted through Google Workspace. Its service may retain submissions in its dashboard. The contact page uses Google reCAPTCHA to help prevent automated abuse; Google may process technical information about your browser, device, and interaction with the spam check. These providers process information under their own terms and privacy practices. We do not add you to a marketing subscription by responding to an inquiry.
Appointment scheduling.
The appointment link opens Calendly, an external scheduling service. It processes the information you choose to enter and may connect your appointment to calendar or meeting services. Calendly’s privacy practices apply on its website. Avoid putting sensitive medical, policy, account, or identity information in scheduling notes.
Website delivery and traffic information.
The website is hosted using OpenAI Sites and its hosting infrastructure. Hosting providers may process technical information such as request details, device/browser information, and traffic metrics to deliver, secure, and operate the website. No additional advertising pixels or marketing analytics scripts are included in this site.
Photographs, fonts, and external links.
Photographs and fonts are served with this website. Links to insurance regulators and other resources open external websites whose privacy practices apply there. Opening a link is optional.
Questions about your information.
Use the inquiry form and select Privacy request to ask about information you have shared, including an access or deletion request. We may need to verify your identity before acting. Applicable rights, lawful recordkeeping duties, and service-provider retention practices may affect what can be provided or deleted.
When services change.
This notice will be updated when inquiry delivery, appointment booking, or other data-processing features are introduced. No marketing subscription is created by viewing the website.
